A few interesting references from Brenton lead off this month.

For an elegant illustration of the effects of respiratory maneuvers and body position on airway caliber refer to:

and finally a new twist on different strokes for different folks.

My own sojourn to the library yielded a rather eclectic collection of references that I thought could be stimulating. It was interesting how the first couple of journals scanned influenced the balance of the scanning. Papers I paused over seemed to fall into a couple of distinct topic areas - was this coincidence in terms of what appeared this month or did I predetermine my reading?

One area where there tends to be two schools of thought is pulmonary rehabilitation. One of the problems is outcome measurement. A couple viewpoints that could be helpful here were:

  • An evaluation of Two Approaches to Exercise Conditioning in pulmonary Rehabilitation. E.A. Normandin et al. Chest 2002; 121:1085-1091
  • Power of Outcome Measurements to Detect Clinically Significant Change in Pulmonary Rehabilitation of patients with COPD. J.P.deTorres et al. Chest 2002; 121:1092-1098.

A second issue is how best to exercise people - both for diagnostic tests and for rehabilitation outcomes. Again there is a myriad of options.

  • The Role of Supplemental Oxygen During Submaximal Exercies in patients with Cyctis Fibrosis. E.F.McKone et al Europ Resp J 2002; 20:134-42.
  • Comparison of the peak exercise Response Measured by the Ramp and 1 Minute Step Cycle Exercise Protocols in Patients with Exertional Dyspnoea. S.M.Revill et al. Chest 2002; 121; 1099?1105.

I was pleased to learn hard out exercise is no better for you than medium intensity exercise

  • Effects of Exercise intensity on Cardiovascular Fitness, Total Body Composition and Visceral Adiposity of Obese Adolescents. B.Gutin et al./ Am J Clin Nutr 2002; 75:818-826

but the Johns Hopkins Precursors Study suggests you need to have started exercising in your youth and then continued on. Oh well.

  • Sports Ability in Young Men and the Incidence of Cardiovascular Disease. T.K. Houston et al. Am J Med 2002; 112:689-95.
  • Tennis Anyone? On the Value of Sustainable, Vigorous Activity and Long-term Studies. Am J Med 2002; 112:733-4.

For those interested in Lung Function Assessment a couple of thought provokers appeared:

  • A Comparison of the Validity of Different Diagnostic Tests in Adults with Asthma. C.J.Hunter et al. Chest 2002; 121: 1051-7.
  • Response of Lung Volumes to Inhaled Salbutamol in a Large Population of Patients with Severe Hyperinflation. M.F.Newton et al. Chest 2002; 121: 1042-50.

From Maureen came this offering hot off the press
  • Stability of Frozen Methacholine Solutions in Unit Dose Syringes for Bronchoprovocation. M J Asmuth et al. Chest 2002; 121:1634-7.

One for any of you interested in sleep apnoea and CPAP
  • Efficiency of Cold Passover and Heated Humidification Under Continuous Positive Airway Pressure. W.J.Randerath. Europ Resp J 2002; 20:183-6.

Working on the principle that is important to stop and think, there were a few theoretical papers worthy of perusing

  • The Increase in Allergic Respiratory Diseases. Survival of the Fittest? A.E.Verner. Chest 2002; 121: 1308-16.
  • The Lung and Carbon Dioxide: Implications for Permissive and Therapeutic Hypercapnia. Europ Resp J 2002; 20:6-11.

And a mindbender

  • Fluctuations and Power Laws in Pulmonary Physiology. B.Suki. Am Rev Resp Crit Care Med 2002; 166:130-7.

Finally some cautionary thoughts for those planning long haul flights to Adelaide for the 2003 Meeting - all a bit mind boggling really

  • Controlled Breaks as a Fatigue Countermeasure on the Flight Deck. D.F.Neri et al. Aviat Space Environ Med 2002; 73:654-64.

and I urge you to think things through before going after the cheapest fares

  • Wheel-well and Cargo Compartment Temperatures of Large Aircraft in Flight: Implications for Stowaways. G.Perry. Aviat Space Environ Med 2002; 73:673-6.
